My name is Shirley Booker. This is a Workman Compensation Case. On 2/13/2026 I was working for Central Counties Services from my home office located in Salado, Texas. I was on my way to get in my car to visit a client. I slipped on a rock landed on my left side and suffered fracture of the proximal left humerus. There was persistent inferior subluxation of the humeral head. Accident on 2/13/2026, had surgery on April 7 at Seaton Main Hospital in Austin. I had a total reverse shoulder replacement. I started therapy on 5/12/26.
